Passport Request Info

Thank you for contacting Congressman Jared Moskowitz’s office.  We will do everything we can to facilitate an appointment with Miami Passport Center. Please note that demand is very high at this time and an appointment cannot be guaranteed.

For the Congressman to make an inquiry on your behalf, he will need you to complete our privacy release form and provide proof of travel in the form of a flight or cruise confirmation. Miami Passport will permit us to submit requests 2 weeks in advance of travel and appointments are provided when travel is within 72 hours.

Please be sure to place the following information in the description area at the bottom of the form:

  1. Date of travel:
  2. Is this for your child? What is the child’s name and DOB?
  3. Is it a renewal application?  Was the last passport lost or stolen?
  4. Did you submit an application by mail?  If so, please provide the application locator number.  (You can go to state.gov to obtain the locator number)
  5. Provide place of the child’s birth: state if born in the USA OR city and country if born outside the USA.
