For the Congressman to make an inquiry on your behalf, he will need you to complete our privacy release form and provide proof of travel in the form of a flight or cruise confirmation. Miami Passport will permit us to submit requests 2 weeks in advance of travel and appointments are provided when travel is within 72 hours.

- Is this for your child? What is the child’s name and DOB?
- Is it a renewal application? Was the last passport lost or stolen?
- Did you submit an application by mail? If so, please provide the application locator number. (You can go to state.gov to obtain the locator number)
- Provide place of the child’s birth: state if born in the USA OR city and country if born outside the USA.
